Abstract

[目的]讨接受和承诺疗法(ACT)对晚期胰腺癌病人压力、心理弹性与生活质量的影响。[方法]选择20161—201812月在我院消化科住院的晚期胰腺癌病人122例。由没有参与该研究的人员使用网络随机化方案根据性别和疾病的种类对病人进行分层后分为治疗组和对照组各61例。对照组失访19,疗组失访24,终有效例数对照组42例、治疗组37例。对照组接受常规治疗,疗组接受标准化的8ACT疗和常规治疗。比较干预后8周和20周两组疾病活动、心理应激和生活质量。[结果]预后8周和20周两组视觉模拟感知压力评分、接受和行动问卷(AAQ-)得分及抑郁、焦虑和压力量表(DASS-21)的抑郁、焦虑和压力得分比较差异均有统计学意义(P<0.05)[结论]ACT疗法可改善晚期胰腺癌病人的心理压力。

[Objective] To investigate the effects of acceptance and commitment therapy (ACT) on stress, resilience, and quality of life in patients with advanced pancreatic cancer. [Methods] A total of 122 patients with advanced pancreatic cancer who were hospitalized in the Department of Gastroenterology at our hospital between January 2016 and December 2018 were enrolled. Patients were stratified by gender and disease type using a web-based randomization scheme by a researcher not involved in the study and then divided into a treatment group (n=61) and a control group (n=61). Nineteen patients in the control group and 24 in the treatment group were lost to follow-up, resulting in a final total of 42 patients in the control group and 37 in the treatment group. The control group received conventional care, while the treatment group received a standardized 8-week ACT program and conventional care. Disease activity, psychological stress, and quality of life were compared between the two groups 8 and 20 weeks after the intervention. [Results] Statistically significant differences were observed between the two groups in visual analog perceived stress scores, scores on the Acceptance and Action Questionnaire (AAQ-II), and depression, anxiety, and stress scores on the Depression, Anxiety, and Stress Scale (DASS-21) at 8 and 20 weeks after the intervention (P < 0.05). [Conclusion] ACT therapy can improve the psychological stress of patients with advanced pancreatic cancer.
