Abstract

目的探讨接纳承诺疗法(ACT)对双相情感障碍患者情绪调节的干预效果。方法将60例双相情感障碍缓解期患者随机分为两组,对照组(30例)给予常规药物治疗和心理教育,ACT干预实验组(30例)在对照组基础之上,每周进行2次ACT干预,每次1.5-2h,均干预5周。干预前后对两组分别采用杨氏躁狂量表(YMRS)评估躁狂水平、抑郁自评量表(SDS)评定抑郁水平、认知情绪调节问卷(CERQ)评估情绪调节方式、接纳与行动问卷第二版(AAQ-II)评估接纳承诺疗法干预疗效。结果 ACT干预实验组YMRS、SDS前后测差值评分显著低于对照组(P<0.5),ACT实验组与对照组干预前后CERQ(积极情绪调节策略与消极情绪调节策略)比较差异具有统计学意义,其中积极情绪策略(t=6.24, P<0.05)、消极情绪策略(t=-6.12,P<0.05),AAQ-II差异有统计学意义(t=-10.79,P<0.5)。结论接纳承诺疗法可以进一步缓解双相情感障碍患者的临床症状,有效改善患者的情绪调节水平。

 

Objective: To investigate the intervention effect of Acceptance Commitment Therapy (ACT) on emotion regulation in patients with bipolar disorder.

Methods: A total of 60 patients with bipolar disorder in remission were randomly divided into two groups, the control group (30 cases) received conventional drug therapy and psychoeducation, and the ACT intervention experimental group (30 cases) carried out ACT intervention twice a week for 1.5-2 hours on the basis of the control group, both for 5 weeks. Before and after the intervention, the two groups were assessed by the Young's Mania Scale (YMRS), the Depression Self-rating Scale (SDS) to assess the level of depression, the Cognitive Emotion Regulation Questionnaire (CERQ) to assess the mode of emotion regulation, and the Acceptance and Action Questionnaire (AAQ-II) to evaluate the efficacy of the acceptance commitment therapy intervention.

Results: The difference scores between YMRS and SDS in the ACT intervention group were significantly lower than those in the control group (P<0.5), and the CERQ (positive emotion regulation strategy and negative emotion regulation strategy) between the ACT experimental group and the control group before and after intervention was statistically significant, among which the positive emotion strategy (t=6.24, P<0.05), the negative emotion strategy (t=-6.12, P<0.05), and AAQ-II were statistically significant (t=-10.79, P<0.5).

Conclusions: Acceptance of commitment therapy can further alleviate the clinical symptoms of patients with bipolar disorder and effectively improve the level of emotional regulation in patients.
