目的 探讨接纳与承诺疗法(acceptance and commitment therapy,ACT)在喉癌术后患者中的应用效果.方法 便利抽样选取2015年12月至2017年1月在某三级甲等肿瘤专科医院头颈外科行喉癌手术的患者48例,随机分为观察组(n=24)和对照组(n=24).对照组接受喉癌术后常规心理护理,观察组对喉癌术后患者进行ACT干预.使用第2版接纳与行动问卷(the acceptance and action questionnaire-Ⅱ,AAQ-Ⅱ)、心理痛苦温度计(distress thermometer,DT)和第4版头颈部肿瘤患者生命质量测评量表(the functional assessment of cancer therapy-head and neck,FACT-H&N)分别评价干预前后两组患者的心理灵活性、心理痛苦与生活质量.结果干预后观察组AAQ-Ⅱ、FACT-H&N得分优于对照组,DT得分低于对照组,差异均有统计学意义(均P<0.05).结论 ACT可以有效缓解喉癌术后患者心理痛苦,提高其心理灵活性和生活质量,是一种有效的心理行为干预方法.
Objective: To explore the application effects of Acceptance and Commitment Therapy (ACT) in patients after laryngeal cancer surgery.
Methods: A convenient sampling method was used to select 48 patients who underwent laryngeal cancer surgery in the Head and Neck Surgery Department of a certain tertiary cancer hospital from December 2015 to January 2017. They were randomly divided into an observation group (n=24) and a control group (n=24). The control group received routine psychological care after laryngeal cancer surgery, while the observation group underwent ACT intervention. The Acceptance and Action Questionnaire-Ⅱ (AAQ-Ⅱ), Distress Thermometer (DT), and Functional Assessment of Cancer Therapy-Head and Neck (FACT-H&N) were used to evaluate the psychological flexibility, psychological distress, and quality of life of the two groups before and after the intervention.
Results: After the intervention, the AAQ-Ⅱ and FACT-H&N scores in the observation group were better than those in the control group, while the DT scores were lower than those in the control group, with all differences being statistically significant (all P<0.05).
Conclusion: ACT can effectively alleviate psychological distress in patients after laryngeal cancer surgery, improve their psychological flexibility and quality of life, and is an effective psychological behavioral intervention.
